Request the complete NICU chart
Ask for the full inpatient chart, not only the discharge summary. Include physician, nursing, dietitian, lactation, pharmacy, respiratory, surgery, infectious-disease, and social-work notes; orders; medication administration records; flowsheets; intake and output; and transfer records.
Feeding logs and product identification
Organize feeding orders and administration entries by date and time. Record maternal milk, donor milk, formula, fortifier, parenteral nutrition, volume, concentration, route, interruptions, and changes. Preserve the manufacturer, full product name, lot or batch number when documented, and the facility source.
Do not infer a brand from a generic entry such as “formula,” “HMF,” or “fortifier.”
Birth, gestational age, and prematurity records
Collect prenatal and maternal history relevant to the chart, delivery records, gestational-age assessment, birth weight, Apgar scores, resuscitation, diagnoses, transfer, and the initial NICU plan. Prematurity and the infant's full medical condition are part of any causation review.
NEC symptoms, diagnosis, and staging
Build a chronology of abdominal findings, feeding tolerance, stool findings, vital signs, laboratory results, cultures, diagnosis, staging, consultations, bowel rest, antibiotics, and clinical changes. Keep a parent's recollection separate from the time-stamped chart.
Imaging, surgery, and pathology
Request radiology reports and images, surgical consultation notes, operative reports, pathology, bowel-resection details, ostomy records, complications, and later reversal or reconstruction. The 2026 federal appellate opinion illustrates why feeding alternatives and case-specific warning causation can matter in one selected case.
Discharge and later treatment
Collect discharge diagnoses, nutrition plans, medications, specialist visits, rehospitalizations, growth and development records, gastrointestinal care, therapies, school or support plans, and documented long-term complications.
Expenses, family impact, and boundaries
Preserve itemized bills, insurance explanations, travel, family leave, wage records, home-care costs, and benefit records. Keep complete originals and do not edit or discard chart material because it seems unfavorable.
